Illness
No child is to be sent to school ill. A child is considered ill if one or
more of the following symptoms are present:
♥ temperature of 101°F
or greater
♥ heavy nasal discharge
♥ frequent productive
cough
♥ sore throat
♥ conjunctivitis
♥ diarrhea (3 or more
loose bowel
movements)
♥ undiagnosed skin rash
♥ headache, particularly
if accompanied
by nasal discharge, sore throat,
reddened eyes
♥ abdominal pain
♥ potential communicable
diseases
♥ vomiting
Children will be isolated from their
classmates and you will be called to remove them from the center for all of
the above mentioned symptoms. It is expected that parents will pick up their
children as soon as possible after being called. Dismissal from the center
is at the discretion of the nurse or the site director.
Medication
Prescription medication can be
given if it is labeled with the child's name, date, name of drug, frequency,
dosage, directions, and physician's name. The parent must also complete a
medicine request form authorizing administration of medicine and specifying
times of administration. Information of the medicine label must agree
exactly with the information on the parent medicine request form. Medicines
administered will be noted with the time and initials on the parent report
and medicine report by the person administering the medicine.
Health and Records
Please be sure that all records, especially
health, are complete and up to date. It is important to notify the center of
any changes in phone numbers or addresses. Please call the center if your
child is to be absent for any reason.
